CONSENT

By booking appointment you hereby state that, to the best of your knowledge, you will provide correct answers in your intake forms provided by the booking software, that includes your name, phone number, address as stated in your state ID or Driver’s License, and any health conditions.

You agree and consent to assessment and treatment. 

You understand that your personal health information will be collected.

You understand that all information that you may provide will be kept confidential unless required by law. 

You understand and consent that your medical information may be shared by the various care providers involved in your care and treatment.

Cancellation requests may be submitted by phone, email, online (through confirmation system) or through text message (561) 465-6063. 

Zensational Massage & Bodywork is committed to providing exceptional service in a timely manner. Unfortunately when a customer cancels without giving enough notice, it prevents another customer from being served; for this reason a No Show/Late cancellation Fees are in effect 

Cancellations must be made within 24-48 hours before your scheduled appointment or you will be charged a 50% fee of the total retail value of your session for the first offense, 100% thereafter; 

**Failure to pay will prevent you from scheduling any future appointment until fee is paid in full. 

If you’re running late, you agree to communicate via text or phone call. You will still be required to pay the full price of the service and you may only receive the remaining time of your service.
